In the event of any accident, injury, illness or death that in anyway relate to myself/child’s voluntary participation in TMHA-related hockey events, associated travel to and from games and tournaments, and any incidental team related activities, we hereby agree on behalf of myself/child to forever waive, forgo and release any and all potential claims for damages, liability, costs and expenses of any kind against TMHA, SRLL, OMHA and Essa Township (the “Releasees”), including all current and former agents, volunteers, employees, officers and directors acting on behalf of the Releasees. We also confirm that as a condition of play at least one parent/guardian within our family has completed the OMHA Respect in Sport training module applicable to parents.
